UConn PCS students are required to complete the forms listed below prior to participating in the program. All forms are included under the Checklist Items on the Status Portal and must be completed by June 10th. A green check next to each item will be indicated when it has been successfully completed.

The following forms are required:

• Health History and Consent for Treatment
• Participant Departure Form
• Program Policies, Consents, and Liability Waiver
• Roommate Request (optional)
• Student Photo
