Required UConn Pre-College Summer Forms

Forms 1-3 below are required for UConn Pre-College Summer participants. After being accepted to the Program and enrolling in your course(s), you will receive information regarding how to complete the following forms:

 

Forms to be complete by ALL students:

 

Forms to be complete for students enrolled in Pre-Vet courses:

 

All forms must be completed and uploaded to the participant’s enrollment profile  by Friday, June 14th, 2019. To log in to the enrollment profile, use the student email address and your confirmation code.

After logging into your enrollment profile using your email address and confirmation code, you will see a blue button that says “Modify/Add Forms/Edit Travel Details”. Upon clicking this button, you will be directed to edit necessary information including uploading a student photo and editing travel details and parent/guardian information. After clicking through a few of these information pages, you will come to a page to upload necessary forms.
